Can anyone tell me HOW a wheel would get out of alignment?Assuming everything is properly adjusted for riding.All bolts and axle nut torqued to the correct spec...cotter pin in place.

There's no way a back wheel can come out of alignment under normal circumstances, assuming it was IN alignment in the first place. Finally got around to checking mine the other day. Had to back off the right adjuster bolt by three flats to get things straight. Not enough of a difference to be noticeable on the hashmarks, but the rear was off-centered by about 1/8" with both parallel.
